Edinburgh Health & Social Care Partnership have contributed the below details on their model of Anticipatory Care Planning in the city. With Thursday of Care Home Week 2018 focusing on Change and Adaptation, the authors wish to highlight how ACPs can help residents and staff change and adapt as their circumstances and care needs alter.
