Care Creates… Collaboration That Works for People and Communities
Scottish Care is calling for stronger integration and meaningful collaboration at the heart of Scotland’s social care support system – a core theme of the Care Creates… campaign. When health, social care, communities and providers work together with trust and shared accountability, care becomes more responsive, more sustainable and more human.
The Scottish Care Manifesto sets out clear, practical actions to move beyond fragmented systems and create joined‑up support that works for people, families and communities.
The manifesto highlights key priority actions to strengthen integration and collaboration across Scotland’s care landscape:
- Mandatory independent sector representation on all Integration Joint Boards, ensuring decisions are informed by frontline expertise, lived experience and operational insight.
- Regular, structured cross‑sector dialogue, creating space for honest conversation, shared learning and collective problem‑solving between statutory, independent and third sector partners.
- Co‑production of care strategies, embedding the voices of people who draw on support, frontline staff and community organisations in the design and evaluation of integrated services.
- Joint training and leadership development programmes, building mutual understanding, shared values and collaborative leadership across health and social care.
- Clear shared accountability frameworks, with transparent roles, responsibilities and reporting to support collective ownership of outcomes.
- Learning from and scaling successful local collaborations, such as the Fife Care Collaborative and Granite Care Consortium, so good practice becomes the norm, not the exception.
- Investment in digital tools and data‑sharing agreements, enabling seamless coordination, timely communication and continuity of care across organisations.
These actions are essential to building a social care system where collaboration is not an added extra, but the foundation of how support is planned and delivered. By breaking down silos and building trust between partners, integration becomes a catalyst for better outcomes, stronger communities and care that truly connects around the individual.
